The bridging the gaps program: three decades of collaborative service-oriented learning in the health professions.
This article describes the long-term impacts of the Bridging the Gaps (BTG) program, a collaborative initiative between academic health institutions and community organizations established in 1991. The study analyzed feedback from program alumni and community partners. Among the alumni surveyed, a significant majority reported that the program broadened their understanding of health issues faced by vulnerable or economically disadvantaged populations (91.8%) and increased their interest in working with these populations (89.2%). The program also had substantial impacts on participants' clinical practice (91.0%), professional roles (93.9%), and research careers (58.7%). Community partners overwhelmingly found the partnership with the BTG program beneficial (97.2%). Many reported that BTG students brought previously unavailable resources to their organizations (67.1%) and that the program strengthened linkages between their agency and other organizations (77.1%). The authors highlight the BTG program as a model demonstrating growth and sustainability in integrating training on social determinants of health through service-oriented learning into health professions education.

Addressing unmet basic needs for children with sickle cell disease in the United States: clinic and staff perspectives.
The purpose of this study was to assess pediatric hematology clinic staff's perspectives regarding barriers and facilitators in addressing unmet basic needs for children with sickle cell disease. Six focus groups were held at urban pediatric hematology clinics in the Northeastern region of the U.S. Four themes emerged: families of children with SCD have numerous unmet basic needs; clinic staff felt they had a role to play in addressing these needs; staff felt their ability to address these needs depended upon caregivers' capacity to act on staff recommendations; clinic staff's ability to address these needs was limited by organizational and systemic factors beyond their control. These findings have important implications for how best to address adverse social determinants of health for this vulnerable pediatric population so that urban-based pediatric hematology clinics can more equitably support families.

Implementing social determinants of health screening at community health centers: clinician and staff perspectives.
Screening for social determinants of health (SDOH) during primary care office visits is recommended by pediatric and internal medicine professional guidelines. Less is known about how SDOH screening and service referral can be successfully integrated into clinical practice. SDOH screening and referral care models can help support the mission of community health centers by identifying unmet material needs. Additional support for SDOH models might include piloting the SDOH screening model workflow and formalizing the workflow before implementation, including the specific roles for clinicians, staff, and patient navigators.
