EvidenceNOW established seven regional cooperatives composed of public and private health partnerships and multidisciplinary teams of experts in 12 States. Bringing together the skills, experiences, and resources of academic researchers, local organizations, and consumer groups, the cooperatives designed interventions to expand primary care practices’ capacity to use evidence to improve the care they provide, including heart health care.

Each cooperative successfully recruited and engaged 200-250 small- and medium-sized primary care practices and provided quality improvement support services usually not available to the practices because of their size. The services included onsite practice facilitation and coaching, expert consultation, shared learning collaboratives, electronic health record support, and data feedback and benchmarking.

Since 2016, EvidenceNOW has reached over 5,000 primary care professionals delivering health care to approximately 8 million people in a range of metropolitan and rural settings.
