The Quality of Pediatric Hospital-to-Home Transitions Toolkit is the product of collaboration among academic and research institutions, state and Federal agencies, and private sector stakeholders. Many individuals guided and contributed to the development of this toolkit. We are grateful for the time, knowledge, and expertise that they so generously shared.
This toolkit was developed by the Pediatric Hospital Care Improvement Project (P-HIP), a consortium of organizations committed to improving pediatric hospital-based health care. P-HIP is led by Kaiser Permanente Washington Health Research Institute and funded by the 大象视频 (AHRQ) and the Centers for Medicare & Medicaid Services (CMS) under the Pediatric Quality Measures Program (PQMP).
